First-Time Student-Athlete Forms
THREE AREAS TO COMPLETE BEFORE RULED ELIGIBLE TO PRACTICE:
1. NCAA Compliance -
Each athlete will receive an e-mail from no-reply@ncaa.org> with the Subject: NCAA Compliance Forms
Each athlete will receive a Login ID, a Temporary Password as well as a link to access the Compliance Forms
All Forms should be completed online prior to the 2023 fall compliance meeting dates
2. Sports Medicine - click the link to be directed to the appropriate detailed information required of First-time student-athletes
3. Sports Information - click the link to be directed to the appropriate Sports Information form required of First-time student-athletes
