Hilbert College Athletics utilizes a program called Sportsware to house each student-athletes medical forms and insurance information.

NYS SICKLE CELL RESULTS REQUEST FORM
Effective August 1, 2022,
all incoming NCAA Division III student-athletes who are beginning their initial season of eligibility and students who are trying out for a team, as part of their required medical examination or evaluation, must obtain or be tested for sickle cell trait and provide the institution with proof of a prior test. NCAA ByLaw 17.1.5.1 Sickle Cell Solubility Test. This is required only once in your lifetime at Hilbert College.
Every student-athlete must submit an updated participation sports physical each year, dated no more than six (6) months prior to the start of the s-a first practice.
The Fall sport season begins practice in early to mid-August while the Winter and Spring sport season programs can begin gathering for non-traditional practice sessions as early as the first week in September.
There are two (2) steps each student-athlete must complete each year to satisfy the Sports Medicine component of NCAA eligibility.

UPLOAD your completed
Participation Physical Form (click link above) to the Sportware portal.
- please note: this form is required for participation in Division III athletics.
- First Time Hilbert students: The Hilbert College Accepted Student Packet all first time Hilbert students receive requires a different physical form so please bring both forms to your doctor's appointment.
- You will then only upload the Participation Physical Form found on this page to Sportsware.

This is strictly an online data capture so each student-athlete must play an active role in submitting | uploading information.
- Please be sure to read and follow each step of the instructions carefully.
- Save any user name and password.
- This will be your only way to submit forms to the Hilbert College Sports Medicine staff each year.
